چکیده

A male child aged 4 years and 5 months, born in Salvador, Bahia. The patient was diagnosed with a complex heart disease with univentricular physiology with following features: main ventricle: the left ventricle; significant pulmonary valve and subvalvar stenosis, partial anomalous connection of right superior pulmonary veins to the superior vena cava, and a wide ostium-secundum-type interatrial communication without previous surgical treatment. The patient underwent total cavopulmonary operation (Fontan type operation) using crimped bovine pericardium extracardiac conduit (size 21) without cardiopulmonary bypass (CPB) support.

Venous cannulation for extracorporeal circulation after total cavopulmonary connection with extracardiac conduit.

We report two cases of venous cannulation after a total cavopulmonary connection (TCPC) with extracardiac conduit. Venous cannulation was performed via side graft sutured to the extracardiac conduit in an end-to-side fashion. The first case was a 3-year-old girl, who suffered from an atypical inferior vena cava obstruction after TCPC. The obstruction region was distal to the anastomosis site of...

eComment: Re: Do we need fenestration when performing two-staged total cavopulmonary connection using an extracardiac conduit?

Between August 1999 and December 2007, 72 consecutive patients with single ventricle physiology underwent a modified Fontan procedure after a bidirectional Glenn shunt using an extracardiac polytetrafluoroethylene conduit without fenestration. Nitric oxide gas inhalation was commenced just after cardiopulmonary bypass together with intravenous phosphodiesterase III inhibitor administration. Aft...

Experimentally modified Fontan circulation in an adolescent pig model without the use of cardiopulmonary bypass

BACKGROUND The feasibility and the hemodynamic outcome of Fontan circulation, without the use of cardiopulmonary bypass, were studied on a beating heart of an adolescent pig model, using a modified total cavopulmonary connection. MATERIAL/METHODS Eight open-chest anesthetized pigs underwent a successful total cavopulmonary connection with the use of an appropriate Y-shaped Dacron-type conduit...

Extracardiac conduit in total cavopulmonary connection

Objective: This study evaluates a new technique of an extracardiac conduit in total cavopulmonary connection in complex congenital heart disease. Methods: Between May 2000 and October 2002, 18 extracardiac conduit surgeries were performed. The patients’ weights ranged from 11 to 29 kilograms, the ages ranged from 1 to 12 years old and 10 patients were male. There were eight patients with tricus...

Appropriate route selection for extracardiac total cavopulmonary connection in apicocaval juxtaposition.

BACKGROUND A malpositioned heart with apicocaval juxtaposition may complicate the management of patients with functional single ventricles when total cavopulmonary connection is performed. We reviewed our experience with extracardiac total cavopulmonary connection in patients with apicocaval juxtaposition with a special focus on route selection and outcomes. METHODS Of 68 patients who underwe...
